How to complete the application

Although the form seems straight forward, there a few areas that are commonly missed. To ensure that your application processes quickly, please be sure to refer to these instructions. 

You can enter most of the information directly on your computer. You can also print out the application, and then fill in most of the required information, however the area for "amount to pay" is a pull down menu choice which is not available when you print out the application. So we recommend that you fill in as much as you can on your computer, then print out the form. . The most commonly missed field is the "Amount to be Charged" and we recommend that you manually write your choice on the form. There are four choices which you can see below. 

IMPORTANT - If you are a member, you may want to double check your membership status before submitting the form.  Please Remember to sign on both pages of the application.

After completing and signing the form, you should scan or photograph the form to email it to us.
